X12 194 — Grant or Assistance Application
The 194 documents a grant or financial assistance application from an organization to a federal, state, or foundation funding agency.
Purpose
The 194 is used in Grants.gov, SAM.gov (federal), state grant portals, and private foundations to transmit a structured application: applicant organization, proposed project, budget, period, deliverables, electronic signatures.
It integrates with the SF-424 (Standard Form 424) ecosystem and Common Federal Grant Standards. Acknowledged by 997; the business response is an award letter (often non-EDI) or a 194 follow-up (request for additional information). Post-award funding adjustments flow through 521 (Income or Asset Offset).
Envelope structure
The 194 travels within the three X12 envelope levels (ISA/IEA,
GS/GE with functional group GR, and ST/SE).

Common pitfalls
- SAM.gov registration: every federal grant requires an active UEI (Unique Entity Identifier) in SAM.gov; a 194 without a valid UEI is rejected before substantive review.
- Indirect cost rate: the indirect rate must follow a Negotiated Indirect Cost Rate Agreement (NICRA) or the 10% de minimis; a fabricated rate routes the 194 to OIG audit.
- Period vs award date:
DTM*RTis the requested project period; the effective award date may differ if the agency defers the start. Do not confuse with the submission window (FOA close date).
