INS01 = subscriber indicator (Y/N), INS02 = relationship code (18 = Self, 01 = Spouse, 19 = Child), INS03 = maintenance type (021 = Addition, 024 = Cancel/Termination, 030 = Audit/Compare) and INS04 = reason. It opens loop 2000 of the 834.
